New Medical Clinic Expansion

We are so excited to announce that this month our Medical Clinic will expand to offer full-time, Monday-Friday service to our neighbors. In partnership with Catherine’s Health Center and Exalta Health, we offer medical check-ups and professional care at our facility, charged through your insurance company. While patients do not need insurance to receive services, most Healthy Michigan Medicaid plans are accepted. We do not turn anyone away, regardless of their ability to pay. There is a sliding fee scale based on income level for out-of-pocket costs.

Services include preventative health screenings, sick visits, chronic disease management, and women’s health exams. These holistic healthcare services are provided in a family practice setting. The doctors at Catherine’s and Exalta specialize in preventative medicine, meaning they strive to prevent illness and reduce health risks.

The expansion of the Medical Clinic at Streams of Hope is part of our mission to foster healthy schools, families, and neighborhoods. Physical wellbeing is a key component of a person’s health, and providing care close to home is essential.

How to Make an Appointment

Visits are available by appointment only at this time. To schedule your first appointment, call Catherine’s Health Center at 616.336.8800. Their offices are open Monday – Friday from 8am –  4:30pm.

Streams of Hope is located at 280 60th St SE. It is located near the following bus stops:

  • Route SL (northbound) – 60th Street Station, Stop #203
  • Route 1 (southbound) – Division and 60th, Stop #206
  • Route 1 (southbound) – Division and Peony, Stop #211
